Fakhіvtsі Nalog.nl developed an online form for issuing an international certificate (certificate) of an individual for the citizens of Ukraine or their children, so as not to waste a foreign (biometric) passport.
What needs to be done?
- Submit an application for a certificate (certificate) on our website.
- Go for otrimanim possilannyam, zavantazhit and rozdrukuvat certificate (certificate), if it is possible (as if it is possible today, then just tell the consul about those who have filed the application - you know it by e-mail).
- For a skin application / person 2 photographs with a size of 3,5x4,5 cm. Documentation is impossible without this photo.
- Come to the reception to the Ukrainian consul at Gaazi. View the photo, paste it and put the seals on the International Certificate (certificate) of the individual.
With this certificate (certificate) you can turn to any sovereign authority of the Netherlands: the municipality (gemeente), the immigration service (IND) and other authorities.
! Respect! The first column (application + certificate) is for you to obtain a certificate. Another column (certificate) – for volunteers who help the consul to obtain certificates directly from the consulate.
! After completing the form and generating the certificate, the information is removed from the server. Data is withheld from the consulate and available only to the consul

Application for an International Identity Certificate for Ukrainians
